DOCUMENT TO BE COMPLETED FOR BODY DONATION TO THE MIGUEL HERNÁNDEZ UNIVERSITY OF ELCHE IN CONNECTION WITH TEACHING AND RESEARCH

Once the donation document has been completed, with the data and signatures of the donor and witness, it must be delivered/sent to the UMH together with the photocopies of the DNI/Passport of both.
The delivery address is: Avda. de la Cadena 33-47 03550 Sant Joan D’Alacante (Alicante)
In order to register the donation, the original document is necessary and therefore, if you are going to send it, it must be by mail.
